Registration Forms

If you would like to register your child with Asbury Community Christian Preschool please complete the following forms and mail or drop off at the school.

• 2012-2013 Enrollment Application

When filling out the enrollment application, please mark 1st, 2nd, 3rd and even 4th choice if possible. In the event that some classes are already filled or do not receive sufficient enrollment, we will be able to place your child in the alternative class of your choice. You will be notified the week of February 7th of placement.

Extended Care

Extended Care classes are available for children who are enrolled in the 3's, 4's or Pre-K classes. Children must be toilet trained.

• Health Inventory Form
• Medication Administration
• All About Me
• Discipline Policy

Registration Fee

A $75.00 non-refundable registration fee per child ($100.00 family max.) is due with the registration form.
